AVA Employer of Choice (EOC) Accreditation Complaints Form

Any party who believes a practice has breached the AVA Employer of Choice (EOC) Accreditation Standards or does not agree with a practice’s AVA EOC Accreditation, may notify the AVA EOC Accreditation Team via the form below. Complaints or concerns may be raised by members of the AVA, those working within the profession, or those who currently do, or have previously worked within the organisation that has become AVA EOC Accredited. The criteria on the form must be met for the complaint to be considered genuine. If a complaint does not satisfy the criteria, there will be no further action taken.

Notification of complaints is kept confidential. Information shared with the AVA and the AVA EOC Accreditation Team in relation to a breach of the EOC Accreditation Standards will not be provided to the practice named in the complaint. Matters where a practice may be in breach of obligations outside of EOC should be referred to the AVA directly via members@ava.com.au.

All complaints made in relation to a breach of EOC Accreditation Standards will be investigated. During this investigation, the practice subject to the complaint may be required to submit a formal statement outlining their adherence to and evidence demonstrating compliance with the EOC Accreditation Standards.

The AVA EOC Accreditation Team will assess complaints and/or concerns in a fair and impartial manner. The AVA EOC Accreditation Team will consider points raised by the practice and complainant in determining whether the practice has breached the EOC Accreditation Standards.

At the conclusion of this investigation, the AVA and AVA EOC Accreditation Team may take one or more of the following actions:

• Allow the practice to retain their AVA EOC Accredited status,
• Allow the practice to retain their AVA EOC Accredited status under special conditions,
• Assist the practice in resolving the issue by providing appropriate recommendations,
• Require the practice to provide evidence of adherence to the EOC Accreditation Standards,
• Vary the Accreditation status of the practice, or
• Revoke the Accreditation status of the practice.
