Branch Formation/Activation Request Form
To assist with scheduling and approval of Branch activities the Branch formation/activation request form has been developed in line with the constitutional requirements for forming and maintaining a Branch (Constitution clauses 21.1, 21.2, 21.4). To assist with completing this form Branches are requested to provide the full name, AVA member number and email address for the seven (7) members and the nominated Branch representative via the below form.
Please note the form must be completed in one sitting and progress cannot be saved.
