All AVA Special Interest Groups, Divisions and Branches must submit an event request form for consideration by the Education Advisory Committee (EAC) to hold an educational or social event as part of the annual AVA events calendar. Applications are considered for approval at the bi-monthly EAC meetings.

Please note the below form must be completed in one sitting and progress cannot be saved.
